Rebecca Dent High Performance Dietitian Ltd collects this information in a variety of ways. Data is collected for example from; an email sent by you to request a nutrition consultation via my ‘contact me page’ on High Performance Dietitian’s website, forms completed by you (e.g. an online pre-consultation form), correspondence with you during nutrition consultations, sms/phone/email communications, from assessments such as body composition measurements, via blood result document from your Doctor or training plans or dietary monitoring apps via an online system.

From other healthcare professionals or organisations:

Rebecca Dent High Performance Dietitian only seeks information from third parties IF required and with your consent only, for example from your GP/Doctor, coach, or other health professional (e.g. physiotherapist, psychologist).

Other Third Parties:

For example online software monitoring systems of food intake and training logs.

How will your information be kept confidential and secure?

Data will be stored in a range of different places, including in your personnel file that is kept in a secure electronic file and/or locked secure filing cabinet, in the company’s IT systems (including the company’s email system and online practice management software).

Why does High Performance Dietitian Ltd process personal data?

The companyneeds to process data that will enable the dietitian to provide healthcare for which you are seeking. High Performance Dietitian will specifically use your information to formulate a nutrition plan and provide lifestyle guidance tailored to your individual needs and requirements.

Some special categories of personal data, such as information about health or medical conditions and treatment received, race/ethnicity, nationality is processed in order to provide the most appropriate nutrition information for you.

You are entirely free to decide whether or not to provide such data and there are no consequences of failing to do so.

Who has access to data?

Your information may be shared internally with other staff members. The company does not share your data with third parties unless consent is gained from you.

Your information may be required to be shared with other health professionals as a referral process or to discuss interventions carried out by other health professionals/Doctor that may impact nutrition advice provided. However again this process will not occur without your formal consent.

If you are an Uphill Athlete Client your data may be transferred to countries outside the European Economic Area (EEA) to share information with fellow Uphill Athlete staff members. Data is transferred outside of the EEA on the basis of sharing relevant information with Uphill Athlete coaching staff only.

How does High Performance Dietitian protect data?

The companytakes the security of your data seriously. All information gathered remains strictly confidential and is not shared with any third parties unless you have given written consent.

The company has internal policies and controls in place to try to ensure that your data is not lost, accidentally destroyed, misused or disclosed, and is not accessed except by its employees in the performance of their duties. Rebecca Dent Registered Dietitian is the only staff member with access to data. All controls in place to protect data are password protected/locked and any online system used has been checked that it follows procedures as per GDPR regulation.

Where the companyengages third parties to process personal data on its behalf (such as  online practice management software), theses companies are under a duty of confidentiality and are obliged to implement appropriate technical and companymeasures to ensure the security of data. All third parties used to process your personal data have been checked to ensure they follow appropriate GDPR regulations.
